- Copy of child's Birth Certificate
- Copy of child's Social Security Card
- Copy of child's Immunization Record
- Copy of child's last report card or withdrawal from previous school
- Proof of Residence in Orleans Parish (utility bill, voter registration, driver license, etc.) (The NOBTS Housing office can provide a letter to verify residence)
