All students planning to graduate must first apply for graduation. The deadline for December Graduation is September 15th and the deadline for May Graduation is February 15th.

The form will no longer accept submission post the deadline. If you miss the deadline please contact the registrar's office directly as soon as possible. All students submitting their graduation application form after the deadline will be charged a $40 late graduation application fee. 

Please fill out this form accurately; it will be used for the Graduation ceremony and the printing of your diploma.

Doctoral Program Graduates 

DMin, DEdMin, ThM, and PhD students must apply for graduation through their respective doctoral office. Graduation applications for students in these degree programs are available on the Web page for each doctoral program. If you are a doctoral student please do not fill out the above form. Instead, fill out the form provided by your program.

It is mandatory that all PhD students fill out the SED and deliver it to the Registrar's Office before they are allowed to walk for graduation. Please click on the link below, fill out the survey, print it, and either hand deliver or mail the survey to the Registrar's Office.
